Please fill out the form below if you plan on attending the 2013 Field of Dreams Conference. Once submitted you will receive and invoice via e-mail that should be paid prior to the conference. Deadline for registration is Friday, October 11.

Conference Registration Fee -- $200

Table fee for Fair Exhibitors -- $100

The Conference host hotel is the Phoenix East/Mesa Hilton Hotel located at 1011 West Holmes Avenue, in Mesa, Arizona 85210 (480-833-5555). Click here to reserve a room.
